New Patient Registration

We are currently welcoming new patients. To register as a new patient you will need to live within our practice boundary. 

How to register

If you would like to register with our practice please download the registration form and email this to our reception team who will process this and respond within 7 days.  Alternatively, a registration form can be collected in person from the practice and the reception staff can help you.

You will need to visit the practice with the following information:

• Medical card
• 1 x photo identification (for example passport or driving licence)
• 1 x utility bill or bank statement showing your current address dated in the last three months

Accountable Named GP

All patients registered with us have a named doctor who has overall responsibility for your care and support. Your registered GP is also your named accountable and your allocated GP. Your allocated GP will be responsible for the provision of your healthcare. If you are unsure who is your named Doctor, please ask at reception. New patients are allocated a Doctor on registration.
